Abstract: Background Udar Roga is one of the grave diseases described in Ayurveda and is considered among the Ashta Mahagada (eight major disorders) due to its chronicity, complications, and difficult management. Jalodara, a subtype of Udar Roga, closely resembles ascites described in contemporary medicine. Ascites is characterized by abnormal accumulation of fluid within the peritoneal cavity and commonly develops as a complication of liver cirrhosis, portal hypertension, chronic liver disease, malignancy, tuberculosis, and cardiac disorders. While modern medicine manages ascites primarily through diuretics, salt restriction, paracentesis, and liver transplantation in advanced cases, recurrence and associated complications remain common. Ayurveda offers a comprehensive therapeutic approach through correction of Agni, elimination of accumulated Doshas, restoration of Srotas patency, and enhancement of metabolic functions. Objective To evaluate the clinical efficacy of Ayurvedic treatment in a patient suffering from Jalodara (Ascites) and assess improvement through clinical, anthropometric, and ultrasonographic parameters. Materials and Methods A 52-year-old male patient diagnosed with ascites secondary to chronic liver disease was treated with a combination of Deepana-Pachana, Nitya Virechana, Mutrala Chikitsa, hepatoprotective Ayurvedic formulations, and dietary modifications for 45 days. Clinical symptoms, abdominal girth, body weight, pedal edema, and ultrasonographic findings were assessed before and after treatment. Results Substantial clinical improvement was observed. Abdominal girth reduced from 108 cm to 94 cm, body weight reduced from 72 kg to 64 kg, pedal edema completely subsided, appetite improved significantly, and ultrasonography demonstrated marked reduction in ascitic fluid. Overall quality of life improved considerably. Conclusion Ayurvedic treatment showed encouraging results in the management of Jalodara. The integrated therapeutic approach effectively reduced fluid accumulation, corrected digestive dysfunction, and improved patient well-being. Further large-scale clinical studies are necessary to validate these findings.
