Triglyceride-rich lipoproteins (TRLs) and their remnants promote atherogenesis through endothelial dysfunction, inflammation, and oxidative stress. Molecular disruptions involving lipoprotein lipase (LPL), apolipoprotein C-III (ApoC-III), and angiopoietin-like proteins (ANGPTLs) impair lipid metabolism and lead to the accumulation of atherogenic particles. This review highlights the mechanistic pathways linking hypertriglyceridemia to cardiovascular risk and explores emerging therapeutic strategies targeting these molecular drivers.
